What is the error ?

95 y. old female presented  with intertrochanteric fr. rt. femur 6 months back. She was operated the second day with DHS and was discharged after 3 days.

6 months later she felt svere pain at the site of the operation. X-ray revealed failed implant with the following x-ray.

Now She is prepared for THR.

What was the error of this fixation ??
