Collapse of scaphoid & lunate

39 years old female complaining of pain and swelling rt. wrist joint since 10  years  . She had no history of trauma.

Pain is intermittent with inability to do her daily life activity during attacks which last for few days.

On examination , the wrist is swollen , minimal tenderness and reasonable range of active and passive movement.

X-ray showed complete collapse of the scaphoid and lunate bones with fragmentation of both,

MRI showed the same features.

WHAT IS THE PROBABLE DIAGNOSIS AND HOW TO MANAGE ?
